Definition:
- Observation with diameter 10-19mm, arterial hyperenhancement and diameter increase by 50% or more on serial exams ≤ 6 months apart
LI-RADS Categorization
- Observations with diameter 10-19mm, arterial hyperenhancement, and diameter increase by 50% or more on serial exams <6 months apart should be categorized: If unsure of diameter increase or arterial hyperenhancement, do not categorize as LR-5g and use table for appropriate categorization
- LR-5g if there is none of the following: capsule appearance, washout appearance
- LR-5 if there is >1 of the following: capsule appearance, washout appearance
Comments:
- OPTN 5A-g is defined in OPTN policy 3.6.4.4 for liver allocation
- Some OPTN 5A-g nodules could also be considered OPTN 5A/LR-5
- OPTN 5A nodules must have all of the following features: diameter of 10-19mm, arterial hyperenhancement, washout and capsule appearance
- Nodules that meet criteria for OPTN 5A-g and also have washout and/or capsule appearance should be considered LR-5
- Note that diameter increase by 50% or more on serial exams ≤ 6 months apart is a subset of threshold growth
- Observations with diameter 10-19mm, arterial hyperenhancement, and diameter increase documented only on exams >6 months apart cannot be categorized LR-5g
